INFUSION PLAY

PARTICIPANT & MINOR RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISK & INDEMNITY AGREEMENT

PLEASE READ CAREFULLY. BY SIGNING THIS WAIVER, YOU ARE GIVING UP CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

This waiver applies to participation in activities at Infusion Play, including Interactive Play, virtual reality (VR), arcade activities, and other games or experiences.

ASSUMPTION OF RISK

I understand that activities at Infusion Play may involve physical movement, physical exertion, virtual reality, interactive games, electronic equipment, moving objects, and other activities that involve inherent risks.

These risks may include, but are not limited to, slips, falls, collisions, dizziness, nausea, motion sickness, muscle strains, sprains, broken bones, or other injuries.

I voluntarily accept and assume the risks associated with participation by myself and/or the minor participant(s) listed above.

I understand that participants are responsible for following all safety instructions provided by Infusion Play staff and for using equipment and facilities appropriately.

EQUIPMENT & FACILITY

I accept responsibility for equipment used by myself and/or the minor participant(s) listed above. I understand that I may be held responsible for damage caused by intentional misuse, reckless behavior, or failure to follow staff instructions.

Infusion Play reserves the right to refuse participation or remove any guest who appears intoxicated or under the influence of drugs or alcohol, presents a safety risk, damages or misuses equipment, fails to follow staff instructions, or significantly disrupts other guests.

Guests removed for these reasons may not be entitled to a refund or rescheduling.

RELEASE & INDEMNITY

To the fullest extent permitted by law, I voluntarily waive and release claims against Infusion Play arising from participation by myself and/or the minor participant(s) named in this waiver.

If I am signing on behalf of a minor, I confirm that I am the minor's parent or legal guardian, or that I am legally authorized to provide this consent on the minor's behalf.

I agree to indemnify and hold harmless Infusion Play from claims, liabilities, damages, or expenses arising from participation by myself and/or the minor participant(s), to the extent permitted by law.

PHOTOS & MEDIA

I authorize Infusion Play to use photographs or electronic media images taken during the visit for promotional purposes. I understand that I may withdraw this permission by notifying Infusion Play.

PURCHASES

I acknowledge that Infusion Play services, packages, tokens, and purchases are generally final sale and are subject to Infusion Play's refund and rescheduling policies.
